Please note: if  your examiner holds an examiner certificate issued in an EASA Member State other than the Netherlands, also include:

• A copy of your examiner's current valid licence.
• A copy of your examiner's current valid examiner certificate.

If so, also include copies of your logbook demonstrating that you have provided at least 10 hours of instrument rating (IR) instruction in the 12 months prior to the expiry date of your flight instructor certificate.

Confirming:

• That you have given 3 hours of practical instruction, which may be flight instruction or MCC instruction in a simulator (FNPT II/III MCC, FTD 2/3 or FFS).
• That the instruction was given under the supervision of an instructor holding TRI, SFI or MCCI privileges, appointed by the training organisation for this purpose.
• How the training organisation assessed your competence during the supervised flight instruction.

Please note: If you completed the training course in another EASA Member State, please also attach the following:

• A copy of the training organisation's Approved Training Organisation Certificate (ATO approval).

• Statement from the training organisation confirming that you have provided at least 3 hours of instruction in an FFS, FNPT II/III or BITD. This instruction must form part of a course for a CPL, IR, PPL, class or type rating.
• Examiner declaration for the revalidation or renewal of a flight instructor certificate, completed and signed by your examiner.

Please note: If your examiner holds an examiner certificate issued in an EASA Member State other than the Netherlands, also include:

• A copy of your examiner's current valid licence.
• A copy of your examiner's current valid examiner certificate.

To demonstrate that you have gained sufficient experience in the 12 months before your flight instructor certificate expires, include at least 2 of the following attachments with your application. The documents you need depend on your specific situation.

• Proof of participation in an FI refresher seminar.
• A copy of your Assessment of Competence (AoC).
• Copies of your logbook demonstrating that you have provided at least 50 hours of flight instruction during the validity period of your flight instructor certificate.
  Or alternatively:
  Examiner declaration for the revalidation or renewal of a flight instructor certificate, completed and signed by your examiner.

For IR instruction, 10 of the 50 hours of flight instruction must include IR or BIR instruction completed in the 12 months preceding the certificate's expiry date.

If you completed your training in an EASA Member State other than the Netherlands, also provide:

• A copy of the training organisation's Approved Training Organisation Certificate (ATO approval).

If your examiner holds an examiner certificate issued in an EASA Member State other than the Netherlands, also include:

• A copy of your examiner's current valid licence.
• A copy of your examiner’s current valid examiner certificate.

To demonstrate that you have gained sufficient experience in the 12 months before your flight instructor certificate expires, include at least 2 of the following attachments with your application. The documents you need depend on your specific situation.

• Proof of participation in a refresher seminar.
• A copy of your Assessment of Competence (AoC).
• Copies of your logbook demonstrating that you have provided at least 50 hours of flight instruction on each helicopter type for which you hold instructor privileges or in a simulator (FSTD) representative of those types. At least 15 of those hours must have been given in the 12 months prior to the expiry date of your TRI certificate.
  Or alternatively:
  Examiner declaration for the revalidation or renewal of a flight instructor certificate, completed and signed by your examiner.

If you completed your training in an EASA Member State other than the Netherlands, also provide:

• A copy of the training organisation's Approved Training Organisation Certificate (ATO approval).

If your examiner holds an examiner certificate issued in an EASA Member State other than the Netherlands, also include:

• A copy of your examiner's current valid licence.
• A copy of your examiner’s current valid examiner certificate.

To demonstrate that you have gained sufficient experience in the 12 months before your flight instructor certificate (SFI) expires, include at least 2 of the following attachments with your application. The documents you need depend on your specific situation.

• Certificate of participation in the SFI refresher seminar.
• A copy of your Assessment of Competence (AoC).
• Copies of your logbook demonstrating that you have at least 50 hours of experience as an instructor or examiner in a simulator (FSTD), at least 15 of which were obtained in the 12 months prior to the expiry date of your SFI certificate.
  Or alternatively:
  Examiner declaration for the revalidation or renewal of a flight instructor certificate, completed and signed by your examiner.

If you completed your training in an EASA Member State other than the Netherlands, also provide:

• A copy of the training organisation's Approved Training Organisation Certificate (ATO approval).

If your examiner holds an examiner certificate issued in an EASA Member State other than the Netherlands, also include:

• A copy of your examiner's current valid licence.
• A copy of your examiner’s current valid examiner certificate.

To demonstrate that you have gained sufficient experience in the 12 months before your flight instructor certificate (IRI) expires, include at least 2 of the following attachments with your application. The documents you need depend on your specific situation.

• Proof of participation in a refresher seminar.
• A copy of your Assessment of Competence (AoC).
• Copies of your logbook demonstrating that you have provided at least 50 hours of flight instruction in the relevant aircraft category as an IRI, within the validity period of your instructor certificate. Of these 50 hours, at least 10 must have been completed in the 12 months prior to the expiry date of your instructor certificate.
  Or alternatively:
  Examiner declaration for the revalidation or renewal of a flight instructor certificate, completed and signed by your examiner.

If you completed your training in an EASA Member State other than the Netherlands, also provide:

• A copy of the training organisation's Approved Training Organisation Certificate (ATO approval).

If your examiner holds an examiner certificate issued in an EASA Member State other than the Netherlands, also include:

• A copy of your examiner's current valid licence.
• A copy of your examiner’s current valid examiner certificate.

To demonstrate that you have gained sufficient experience in the 12 months before your flight instructor certificate expires, include at least 2 of the following attachments with your application. The documents you need depend on your specific situation.

• Proof of participation in a refresher seminar.
• A copy of your Assessment of Competence (AoC).
• Copies of your logbook demonstrating that you have provided at least 75 hours of flight instruction during the total validity period of your instructor certificate, of which at least 25 hours were given per type or class of aircraft within the category. Additionally, at least 25 hours of instruction must have been provided in the 12 months prior to the expiry date of your certificate.
  Or alternatively:
  Examiner declaration for the revalidation or renewal of a flight instructor certificate, completed and signed by your examiner.
