Use the forms below to apply for certificates, approvals and other documents for seagoing vessels in the Netherlands through the Human Environment and Transport Inspectorate (ILT).
You can only submit a change of address using the Application for amendment or duplicate merchant shipping certificate form.

Application Additional certificates for merchant ships
You may use this form for the first issuance, renewal or extension of:
- Minimum Safe Manning Document (first issuance only);
- Condition Assessment Scheme;
- Statement for sailing outside designated area;
- Declaration of Maritime Labour Compliance (DMLC) - Part I;
- Other certificates.
Application civil liability certificate for merchant shipping - seagoing vessels
You may use this form for the first issuance, renewal or extension of the following liability certificates:
- Civil liability for bunker oil pollution damage;
- Civil liability for oil pollution damage;
- Liability for passengers:
- Liability for the removal of wrecks.
Request MLC Substantial Equivalencies and Exemptions
Send this form as an attachment to your application for the Declaration of Maritime Labour Compliance (DMLC) - Part I (form Application additional certificates for merchant ships).
Application Inspection MSMD or MLC Certificate
Use this form to request information on Minimum Safe Manning Documents (MSMDs) and MLC Certificates issued by the ILT.
Recognitions
Application non-ship-related recognitions or type approval equipment
You may use this form to apply for type approvals, recognition of notified bodies and recognition of certification bodies for radio equipment.

The shipper (cargo owner) of the solid bulk cargo requests authorisation for transport from the ILT using the form IMO Solid bulk cargo information reporting questionnaire.
