Use this form to request temporary permission to act as a pilot with a rating or certificate. This permission is valid only if you have successfully completed the required training and testing. The permission is issued under Commission Regulation (EU) No 1178/2011, Annex VI, ARA.FCL.215(d). It extends a valid EASA Part-FCL licence issued by the Civil Aviation Authority the Netherlands (CAA NL).
