Authorisation form

If you would like another person or organisation to submit an application to the Human Environment and Transport Inspectorate (ILT) on your or your organisation's behalf, you can do so using this authorisation form.

Use this signed and completed authorisation form to give the appointed person or organisation permission to prepare an application, to submit an application and to correspond regarding the application on your or your organisation's behalf.

This authorisation form can also be used when another person submits an application to the ILT and you or your organisation intend to pay for it.
